(Al's is great. Also try Buffalo Joe's "cheesy beef") Anyway, one group of people I know finally took up the challenge and opened up an Italian Beef place in LA. I believe it was last year, but I haven't heard an update yet. For #1, I always wondered about that too since most of my friends who tried Italian beef while visiting Chicago loved it.
